I would assume the actor is Onoe Tamizô the 3rd, who held the name of Onoe Taminosuke from 1874 to January 1918. More details here:

For the role, it is Otsuma ... but in which play? One famous Otsuma appears in "Sakuratsuba Urami no Samezaya" (commonly called "Unagidani"). Not sure whether or not it is this Otsuma.
