No, I pretty much agree with everying in your post. I too find that many institutions i) take a lot of liberties with the translation, and ii) don't make clear when they do so.

I mean, for example, I can understand an institution interpolating 'Palace' in "The Battle at Yamaki [Palace] in Izu Province" (since things that would be obvious to Japanese readers might not be so obvious to Westerners), but there is no excuse for not flagging the interplation with the '[]'!
