It's the logo of the Hisakataya (ひさかた屋) Group. The surmono is part of a triptych or polyptych showing a group of leading kabuki actors in the play 'Sukeroku yukari no edozakura' (Sukeroku and the Flower of Relationship in Edo), performed at the Ichimura-za, 2nd lunar month of 1811. The poem at the left is signed by Hisakataya (久堅屋), leader of the group.
The actor here is most probably Matsumoto K˘shir˘ V as Hige no Ikkyű.
Other depicted actors that I know are Ichikawa Danjur˘ VII as Sukeroku and Iwai Hanshir˘ V as the courtesan Agemaki.
