Two small corrections:
-The temple name is most likely read as Jōraku-in.
-The last characters are nijū hachi 二十八, so the person died at the age of 28.


As for the poem, it reads as follows:

Tokiwa-naru 常磐なる
Matsu-no ki kake-o 松の木かけを
Shimeshi mi-o しめし身を
Ima-wa hachisu-no 今は蓮の
Tsuyu-to kienan 露ときえなん

My poor English skills wouldn't do it justice, so I leave the translation to someone else.
